  • CDA of PEI membership is in good standing
  • CCDP certification fee
  • Resume and letter of intent
    • resume tailored to career development practitioner experience
  • Reference form completed
  • Criminal record check
  • Copy of certificates: Fundamentals and Ethics​
  • Photocopy of transcripts​
  • Statement of Understanding / Ethical Code of Conduct
